Dear Parents/Guardians:

The Polk County Health Department will be offering in-school influenza (flu) vaccinations to students at Clear Lake School District on October 14, 2024. All guardians wishing to have their child vaccinated must complete an electronic consent form prior to the clinic day. Polk County Health Department will not be able to accept paper copies. This will allow them to more efficiently prepare for the amount of vaccine, supplies, and staffing needed. No walk-ins will be accepted.

The flu vaccine will be available for grades K-12. They will not be vaccinating 4K students. The standard flu shot and the nasal flu mist will be offered. If you DO NOT want your child to receive the influenza vaccine at the school district, you do not need to take any further action.

Again, all individuals interested in the flu shot must register online. The electronic form will close on October 4th 2024 at 11:59 pm.

Medical Information

If your child must have medication at school please fill out forms 1 and 2 along with additional medication specific forms needed for your student and return in person to the office, do not send to school with your child. For over the counter medication the parent permission slip is required and all medication needs to be in the original bottle. Please contact the office with any questions that you may have.
